Life Policy

 

 

Address Change

 

For change or correction of billing address, fill out the Request For Address Change form and submit to Client Services Department.

 

Request should be filed at least five (5) weeks prior to the next premium due date to facilitate sending of premium notice to the new address.

 

 

 

 

Absolute Assignment

 

Fill out the upper portion of the Assignment / Release of Assignment form and submit to Client Services Department together with a Beneficiary Designation form. Accomplish the Irrevocable Form if you are assigning an irrevocable beneficiary/ies.

 

Court order will be required  if the irrevocable beneficiary to countersign the beneficiary change form is a minor.

 

  

 

Collateral Assignment

 

Fill out the upper portion of the Assignment / Release of Assignment form and submit to Client Services Department.

 

Court order will be required  if the irrevocable beneficiary to countersign the assignment form is a minor.

 

  

 

Release of Assignment

 

Fill out the lower portion of the Assignment / Release of Assignment form and submit to Client Services Department.

 

A certification from the Assignee may also be submitted confirming that the loan has been duly paid. If the assignee is a bank or corporation, a secretary’s certificate should be submitted together with the photocopy of valid ID of the signatories on the release letter/form.

 

 

 

Beneficiary Change

 

If you wish to update your beneficiaries on your policy, fill out the Beneficiary Designation form and submit to Client Services Department. Assign a trustee for minor beneficiary/ies and indicate the relationship to the Life Insured.

 

Accomplish the Irrevocable Form if you are assigning an irrevocable beneficiary/ies.

 

Please note that you are required to submit a Court Order if the irrevocable beneficiary to countersign the beneficiary change is a minor.

 

Letter of explanation may be required if the beneficiary being assigned has no insurable interest (e.g., friend).

 

   

 

Declaration of Loss of Policy

 

In case your policy contract is lost or was torn out due to long period of storage or due to typhoon, you may request for a duplicate copy by filling out the Declaration of Loss of Policy form, have it notarized and submit to Client Services Department.

 

A policy fee of Php142.00 should be paid upon submission  of the form.

 

 

 

 

Dividend Withdrawal

 

Dividends left on deposit to accumulate interest can be withdrawn anytime. It can also be used to pay for premium due or to pay off existing loan. However, for policies with outstanding loan which exceeds the cash value, dividend withdrawal will not be allowed unless used to pay off the loan.

 

Fill out the Request for Dividend Withdrawal form and submit to Client Services Department together with a photocopy of your two (2) valid IDs and your policy contract.

 

 

 

Mode Change

 

There are four (4) modes of payment where you can choose from in paying  your premium due.

  • Annual
  • Semi annual
  • Quarterly
  • Monthly

Fill out the Request for Mode Change form and submit to Client Services Department. If you choose the monthly mode, please submit 12 postdated checks (PDCs) or a Credit Card Authorization form to Billing and Collection Department.

 

You may be required to pay premium arrears depending on your due date and the date of your request.

 

 

 

Addition/Deletion of Rider

 

Fill out the Application for Policy Change form and submit to Client Services Department.

 

For addition of rider, fill out the Reinstatement Form also. Additional documents may be required depending on the initial declaration on the insurability form. Additional premium should be paid plus a policy fee of Php150.00 if the application for addition of rider is beyond six (6) months from Issue Date.

 

 a. Reinstatement Form (for Life)
 b. Reinstatement Form (for Plans)
 c. Reinstatement Form (for Corporate)

 

For deletion of rider, submission of court order is required if the irrevocable beneficiary to countersign the policy change form is a minor.

 

 

 

Change/Correction of Birthdate

 

Fill out the Application for Policy Change form and submit to Client Services Department together with a photocopy of Birth Certificate.

 

Policy contract should also be submitted and premium arrears should be paid, if there is a change in age.

 

 

 

Conversion

 

Fill out the Application for Policy Change form and the Application for Attained Age Conversion form and submit to Client Services Department together with your Policy Contract.

 

New premium and arrears should be paid.

 

A court order is required if the irrevocable beneficiary to countersign the policy change form is a minor.

 

 

 

Dividend Option Change

 

There are several dividend options available for you namely:

  • Pay in cash
  • Towards Premium Payment
  • Left to accumulate interest

Fill out the Application for Policy Change form to change your dividend option and submit to Client Services Department.

 

 

 

Increase/Decrease of Face Amount

 

Application for increase in face amount should be made within six (6) months from issue date and not beyond one (1) year from the Policy Year Date (PYD).

 

Fill out the Application for Policy Change form and the Reinstatement form and submit to Client Services Department together with your policy contract. Additional documents may be required depending on the initial declaration in the insurability form. Additional premiums should be paid effective PYD.

 

 a. Reinstatement Form (for Life)
 b. Reinstatement Form (for Plans)
 c. Reinstatement Form (for Corporate)

 

For decrease in face amount, fill out the Application for Policy Change form and submit to Client Services Department together with your policy contract. Submission of court order is required if the irrevocable beneficiary to countersign the policy change form is a minor.

 

  

 

Name Change

 

For change or correction of name, fill out the Application for Policy Change form and submit to Client Services Department together with any of the following documents:

  • Photocopy of birth certificate, or
  • Photocopy of marriage certificate, or
  • Any other valid proof of identification (e.g. passport, SSS ID, Driver’s License)
  • Additional identification reflecting the alias, if applying for A.K.A.

 

 

Non Forfeiture Option (NFO)

 

There are several non forfeiture options (NFO) available for you namely:

  • Automatic Premium Loan (APL)
  • Reduced Paid-Up (RPU)
  • Extended Term Insurance (ETI)

Fill out the Application for Policy Change form to change your non forfeiture option and submit to Client Services Department.

 

Submission of court order is required if the irrevocable beneficiary to countersign the policy change form is a minor.

 

 

 

Plan Change

 

Application for change in plan should be made within six (6) months from issue date.

 

Fill out and submit the Application for Policy Change form and Reinstatement form to Client Services Department together with your policy contract. Additional documents may be required depending on the data disclosed in the initial documents submitted. Additional premiums may also be required.

 

 a. Reinstatement Form (for Life)
 b. Reinstatement Form (for Plans)
 c. Reinstatement Form (for Corporate)

 

Submission of court order is required if the irrevocable beneficiary to countersign the policy change form is a minor.

 

 

 

Policy Year Date (PYD) Change

 

Application for change in policy year date (PYD) should be made within six (6) months from issue date. The new PYD should not be later than the issue date.

 

Fill out the Application for Policy Change form and submit to Client Services Department together with your policy contract. Premium arrears may also be required.

 

Submission of court order is required if the irrevocable beneficiary to countersign the policy change form is a minor.

 

  

 

Policy Loan

 

Fill out the Policy Loan Agreement form and submit to Client Services Department together with the following documents:

  • Policy contract
  • Photocopy of two (2) valid Identification forms (IDs) such as passport, SSS ID, Driver’s License, etc.
  • Release letter and Secretary’s Certificate from the bank if policy is assigned

Submission of court order is required if the irrevocable beneficiary to countersign the loan form is a minor.

 

 

 

Reinstatement


There are two (2) types of Reinstatement namely:

  • Straight Reinstatement
  • Reinstatement through Redating


Fill out the Reinstatement form and photocopy of valid identification form (ID) and submit to Client Services Department.
Additional documents may be required depending on the initial declaration in the insurability form.

 

 a. Reinstatement Form (for Life)
 b. Reinstatement Form (for Plans)
 c. Reinstatement Form (for Corporate)

 

Submission of court order is required if the irrevocable beneficiary to countersign the reinstatement form is a minor.

 

 

 

Self Liquidating Premium Option (SLPO)

 

Dividends may also be used to pay for future premiums by availing the Self Liquidating Premium Option (SLPO).

 

Fill out the Request for Dividend Withdrawal form and submit to Client Services Department together with a photocopy of one (1) valid identification form (e.g. passport, Driver’s License, SSS ID, etc.).

 

  

 

Surrender for Cash Value

 

Fill out the Request for Surrender for Cash Value form and submit to Client Services Department together with the following documents:

  • Policy contract
  • Photocopy of two (2) valid Identification forms (IDs) such as passport, SSS ID, Driver’s License, etc.
  • Release letter and Secretary’s Certificate from the bank if policy is assigned

Submission of court order is required if the irrevocable beneficiary to countersign the surrender form is a minor.
