No. You will only be asked about your height and weight to know if you are qualified, and confirm your health declaration before availing the plan

How do I file a claim for living benefits?

Simply submit the following claims requirements:

Fully accomplished claimant’s statement form
Copy of Claimant’s Valid Identification Document (Photo-bearing and valid, government-issued with 3 specimen signatures)
Attending Physician’s Statement (APS) for the specific illness being claimed (Should be accomplished by a Medical Specialist along with confirmatory test results)
Complete Medical Records including but not limited to the following:
- Admission History
- Clinical Abstract
- Laboratory Test Results
- Record of Operation
